Kulikovsky type needle (size G-15 diameter d-2.0 mm 1 pc.) is intended for punctures of the maxillary sinus.

Made of stainless steel 85 mm long, curved tip.

Puncture of the maxillary (maxillary) sinus has been considered for many decades one of the most effective ways to treat inflammatory pathology of the paranasal sinuses. In addition, this procedure also has diagnostic value, due to the fact that it allows you to assess the nature of the process and the amount of exudate, as well as determine the patency of the natural sinus fistula, establish the presence of blood in it and suspect the presence of any soft tissue formation here.

Indications for use

Indications for performing a puncture of the maxillary sinus include the absence of a positive effect from conservative therapy for acute or chronic sinusitis; severe condition of the patient, accompanied by headaches and / or pain in the area of the paranasal sinuses; the level of fluid or accumulation of blood in the maxillary sinus (hematosinus) determined by x-ray or computed tomography; obstruction of its natural anastomosis. In addition, a puncture is performed for the introduction of a contrast agent in x-ray diagnostics.

Peculiarities

The procedure itself is quite simple and does not require long preparation. The latter includes a toilet of the nasal cavity and local anesthesia with a solution of 10% lidocaine or 2% dicaine in combination with adrenaline. A puncture is made with a Kulikovsky needle, which is inserted under the inferior turbinate at a distance of approximately two centimeters from its anterior end. The choice of this place for the puncture is due to the fact that it is here that the medial wall of the maxillary sinus has the smallest thickness. In this case, the needle must necessarily be turned towards the outer corner of the eye of the corresponding side.

If the procedure is performed correctly, then when the needle is passed through the thickness of the bone after applying a very small effort, a characteristic sensation of failure is noted. The needle is immersed in the sinus by about five millimeters, in some cases it can rest against the contralateral wall of the sinus, so after performing the puncture, you should make a few gentle, light shaking movements with the needle or aspirate a small amount of content with a syringe. If this can be done freely, then the needle is located directly in the lumen of the sinus, otherwise it has to be displaced a few millimeters outward.
